UDISE+ Data Checklist

UDISE+ (Unified District Information System for Education Plus) is the Ministry of Education’s annual data collection that every recognised school in India must file. The form runs across school profile, physical infrastructure, teachers, enrolment and facilities, and gathering it at the last minute is where schools slip. Use this checklist to collect the right data through the year so your UDISE+ filing is accurate and on time.

School profile & identification

  • UDISE code of the school (11-digit)

  • School name, category and management type (govt / private aided / unaided)

  • Year of establishment and lowest & highest class

  • Board of affiliation for secondary / higher-secondary sections

  • Medium(s) of instruction

  • Rural / urban location, cluster, block and district

  • School email, phone and head-of-school details

  • Pre-primary section availability and management

Physical infrastructure

  • Total number of classrooms and their condition (good / needs repair)

  • Building status (own / rented) and boundary wall

  • Number of functional toilets — separate for boys, girls and CWSN

  • Drinking water availability and source

  • Electricity connection and functional status

  • Library / reading room and number of books

  • Playground, ramps and handrails for CWSN accessibility

  • Computer lab, number of functional computers and internet availability

Teachers & staff

  • Total teachers by gender and by regular / contract status

  • Teacher qualifications (academic and professional) and trained status

  • Subjects and classes taught by each teacher

  • Teachers deployed for non-teaching / administrative duties

  • Aadhaar-linked teacher records (where required)

  • In-service training received during the year

  • Number of non-teaching staff

Enrolment & students

  • Class-wise, section-wise and gender-wise enrolment

  • Enrolment by social category (General / SC / ST / OBC / EWS)

  • Number of Children With Special Needs (CWSN) by type of disability

  • Students receiving free textbooks, uniforms and mid-day meal (as applicable)

  • New admissions, repeaters and transfers during the year

  • Aadhaar / student ID (PEN — Permanent Education Number) coverage

  • Attendance and dropout data

Facilities, grants & results

  • Receipt and utilisation of government grants (for eligible schools)

  • Availability of ramps, medical check-ups and health cards

  • Kitchen / mid-day-meal facility status (as applicable)

  • SMC / SMDC constitution and meetings held

  • Board examination results for classes X and XII

  • Scholarships disbursed and beneficiaries
